This group will provide virtual wraparound services to support patients when they cannot get in with their own PCP - closing gaps in care, ensuring timeless access to important medications and solving urgent clinical needs. We will be serving patients in Louisiana, North Carolina, South Carolina, and Georgia, and we are looking for an MD/DO who practices in one or more of these states. This role will focus on the collaborative relationship through the support of physician assistants and advanced practice registered nurses for approximately 3 hours/month. We provide best in class support for providers from ambient AI documentation tools to best in class patient and population health management tools to make doing the right thing for your patients as seamless as possible. **Responsibilities:** · Act as a collaborating physician for nurse practitioners or physician assistants, including completing regular chart review and coordinating monthly meetings. · Be available to act as a resource for the advanced practice provider to ensure exceptional patient care. · Follow policy and protocol defined by Clinical Leadership. · Meet with clinic leaders about quality of care, review of outcome data, policy, procedure, and records issues. · Assure personal compliance with licensing, certification, and accrediting bodies. **Use your skills to make an impact** **Required Qualifications:** · Current and unrestricted medical license in the state of LA, NC, SC, or GA, prior to the start of employment. If licensed in SC or GA, must also practice physically within the state. · Graduate of accredited MD or DO program of accredited university · Board certification or eligible to become certified (ABMS or AOA) in Family Medicine, Internal Medicine, or Geriatric Medicine · Active and unrestricted DEA license · Medicare Provider Number · Medicaid Provider Number · Excellent verbal and written communication skills · Superior patient/customer service skills · Demonstrate a high level of skill with interpersonal relationships and communications with colleagues/patients · Strong computer proficiency, including responding to emails and navigating EMR interfaces · Expertise with web-side manner and willing to take a sponsored CME course on web-side manner, as needed **Preferred Qualifications:** · Fully on board with the concept of virtual primary care and has 2+ years with engaging in patient-facing telemedicine/telehealth · Knowledge of Medicare guidelines and coverage · Knowledge of HEDIS quality indicators · Experience working in value-based care settings · Experience working as a collaborating physician · Telehealth experience Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required. **Scheduled Weekly Hours** 1 **Pay Range** The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting.
